NOTIFICATION TO INSUREDS

We make every effort to maintain an accurate participating provider list, but it is ultimately your responsibility as the patient to verify with the provider that they are still participating when you make your appointment.

Please be aware that every physician in a group practice may not be participating and that some office locations of a particular physician may not be participating. If additional medical services are needed, such as laboratory tests or x-ray procedures, you should also verify in advance that they are participating.

At the time of your appointment, please request that the provider's office copy your ID card for their file so they will be able to identify the PPO network through which you are accessing their services.

NOTIFICATION TO MVPPO CLIENTS

PPO network access is limited to the insureds' domicile state unless agreed upon in advance by the payor, TPA or insurance company and MVPPO.
